Fill out and complete the documents below:

1. Life insurance application form along with all the attachments
2. Copy of Customer’s and Insured’s identity card
3. Surat Pernyataan Pemahaman Calon Pemegang Polis (statement letter of understanding)
4. First contribution payment slip
5. Copy of Surat Kuasa Debet Rekening (approval for Bank account deduction) for renewall contributions payment
6. Copy of the passbook front page (if the Customers do not have a passbook, it can be replaced with another document that stated the information of the Bank account name and account number)
7. Other additional documents if required

Failure to provide accurate data will result in the following:

1. Contribution refund / cancellation of insurance protection
2. Adjustment of contribution and/or insurance benefits
3. Claim rejection
